abstract: Latar Belakang: Bunuh diri merupakan masalah kesehatan masyarakat yang serius.
Dampaknya meliputi trauma keluarga, kejadian berulang pada keluarga dengan riwayat
bunuh diri, dan persepsi bahwa bunuh diri bukan tindakan yang salah. Gunungkidul
menyumbang kasus bunuh diri tertinggi di DIY dengan total 145 kasus selama 2021–2025,
didominasi kelompok lansia (>55%). Risiko bunuh diri pada lansia diperkirakan
dipengaruhi oleh akumulasi faktor sistem kesehatan, sosial, komunitas, relasional, dan
individu. Penelitian ini bertujuan mengidentifikasi determinan utama kejadian bunuh diri
pada lansia di Kabupaten Gunungkidul. Tujuan: Mengidentifikasi determinan utama yang
berperan pada kejadian bunuh diri yang pada lansia di Kabupaten Gunungkidul. Metode:
Penelitian menggunakan desain case control dengan pendekatan kuantitatif retrospektif.
Populasi terjangkau adalah seluruh lansia berusia ≥60 tahun di Kabupaten Gunungkidul
periode 2021–2025. Sampel terdiri dari 82 kasus dan 82 kontrol yang diambil
menggunakan purposive sampling dengan matching 1:1. Data primer dikumpulkan melalui
kuesioner Instrumen Mitos Pulung Gantung, sedangkan data sekunder bersumber dari arsip
resmi Dinas Kesehatan dan Polres Gunungkidul. Analisis data dilakukan secara univariat
dan bivariat menggunakan SPSS versi 27. Hasil: penelitian menunjukkan gangguan jiwa
berhubungan bermakna dengan kejadian bunuh diri (p = 0,000) dengan prevalensi 56%
pada kelompok kasus. Penyakit kronis tidak menunjukkan hubungan signifikan (p = 0,339),
demikian pula mitos pulung gantung (p = 0,623). Kesimpulan: Determinan utama kejadian
bunuh diri pada lansia di Kabupaten Gunungkidul adalah gangguan jiwa.
